Support FAQ
Support applications are currently open to those living in the Greater San Diego Area, although we plan to expand services in the near future.
We will have no eligibility restrictions around age or level of employment, as we believe all people dealing with a mental health condition are deserving of proper care.
As a part of our application process, we ask candidates to complete a statement of financial need, including financial resources and considerations impacting your ability to access treatment. We do not currently have any income caps to apply, and assess each application on a case-by-case basis.
First of all, thank you for caring for someone in need. When you’re ready to start, click the ‘Apply for Support’ button to begin the application process. You will see a question asking, ‘Are you seeking support for yourself or on behalf of someone else?’ Click on the ‘someone else’ option, and then you will be directed to an application specifically geared toward answering questions about another person. Please complete the questions to the best of your abilities. If you need assistance filling out the form or if you have any questions, please reach us at hello@fundamental.health.
FundaMental Health grants go directly towards services and treatments from FundaMental Health’s network of providers. On a case-by-case basis we can also cover other out-of-pocket costs associated to treatment including deductibles, co-pays, prescriptions, short-term childcare and pet care, and travel to/from treatment.
Applications are submitted online or by phone, and will be reviewed by our staff, including the CEO and members of the Board of Directors, who will then decide which applicants will receive a mental health grant. We will also conduct an in-person or over-the-phone interview prior to awarding a grant. Our goal is to help as many people as we can, but based on limited resources, we cannot accept every application we receive.
We do our best to review and respond to applications within two weeks of submission. If accepted, a grant recipient may begin treatment as soon as they are connected to a provider within our network. This connection typically happens after a diagnostic assessment of the grant recipient.
In some cases, a grant recipient may already have a mental health provider they would like to work with. If so, the FundaMental Health team will conduct due diligence on the preferred provider to ensure they meet and adhere to our high standards of care.

Provider FAQ
Becoming a FH provider grants you access to full, on-time payments, client leads, and support and collaboration from our care coordinators, who work to reduce administrative tasks so that you can focus on helping the client. We also offer the opportunity to be part of an altruistic mission and service client populations who typically can’t access mental health services.
The provider network is a group of mental health providers who operate within FundaMental Health and offer services to our client population.
Range, flexibility, efficiency.
The provider network allows FundaMental Health to access a list of quality providers trained in a wide range of modalities. This enables us to adequately match clients with the best-fit providers, including the flexibility to change providers or services if necessary. Our provider network also streamlines the process of matching clients to providers, ensuring clients receive timely services while providers can assist those in need faster.
During onboarding, you will work with a care coordinator to complete initial paperwork. Once you receive a client, a care coordinator will arrange recurring services that align with your schedule. Then, you will do what you do best and begin the client-provider care journey! At weeks eight and ten of this journey, there will be clinical check-ins to discuss additional authorizations and/or changes in treatment. Ongoing communication will also occur regarding invoices for payments and any other questions you may have as you go.
You will work with clients who meet the criteria for the FundaMental Health support program. Currently, our focus is on youth and LGBTQ+ populations.
There is currently no fixed rate for provider payment. Payment will be determined on a case-by-case basis and discussed during the onboarding process.
Clients are referred to providers based on client availability/schedule, the modality offered, and accessibility.

You are helping the underserved. At this point in time, this includes two populations in San Diego, CA — youth and the LGBTQ+ community. These populations experience disproportionate rates of mental health issues, and FH is here to connect them to critical services thanks to the help of donors like you.
